Information Needed for Enrollment Process

 You must have the following information before you can be officially enrolled according to the board of education.

1. Withdrawal form from previous school

2. Most recent report card

3. Proof of Residence – See List below

• Signed lease agreement

• Rental receipt with address and name of legal guardian or parent

• Utility bill (gas, water, electric, No phone bills

• Current listing on income tax forms

• Home ownership proof

• Personal property tax listings

• Bank statements and payroll addresses to domicile

(A driver’s license is not adequate proof of residence.)

4. Certified Birth certificate or other documented proof of age.

 In place of birth certificate, “2 of the following”:

• hospital record

• physicians record

• midwife record

• religious record from mosque, temple, church,

• parent's affidavit

• school records  

• immigration or naturalization record  

• passport

• census  

• life insurance records

5. Parent identification card as listed on the child’s documented proof of age.

6. Certified Copy of Immunization records / Kindergarten Physical for Kindergarten enrollments (we will give you 30 days to get this to us)

7. Custody papers (if you are not the biological parent we must have written documentation signed by a judge)

8. Attendance Report

9. Individualized Education Plan or 504 Plan if in Exceptional Children Program

The following steps will be taken by Pine Level Elementary School to complete the enrollment process.

1. Confirmation from the bus garage stating that the residence is in our school district

2. Confirmation from previous school that there is no bad behavior, attendance or custody issues that we should be alarmed about

3. All other academic information
