All athletic forms are on "FinalForms", which is an online site that we use to manage athletic paperwork & physicals.  Here is a video link on How to Create an Account in FinalForms & Add a Student in FinalForms.

*Parents must create an account in FinalForms & complete all forms online in order for their child to be eligible to tryout for any sport.

*Parents must Upload a copy of the student's completed athletic physical form into FinalForms.  Here is a video link on How to upload a physical into FinalForms

*Parents & Students must RE-SIGN FORMS in FinalForms every year. Here is a video link on How to Update your forms in FinalForms

Students interested in trying out for a sport must have minimum of a "D" average for the previous semester and must pass at least 3 of the 4 core classes from the previous semester. Students also need in good behavioral standing with the school. Those expectations along with having FinalForms completed are required in order to be eligible to tryout.

JCPS parents can request access to the Parent Portal by completing the Request for Parent Portal Access Form. Please be sure to provide the most accurate information (full name, phone number, and email address) when completing the form. It will take 3-5 business days to process the request.
