It is the parent's responsibility to plan worthwhile objectives and activities for a family sponsored educational trip. The information on this form must be completed in full by the parent/guardian. This form must be submitted five days prior to the trip. Prior approval should be obtained BEFORE absences can be excused. You may only sponsor 1 trip per year.
